We would be pleased to have you register as a parishioner of Our Lady Queen of Heaven Parish.This link will lead you to our parish census form. While we are not set up to receive it electronically, you can print a .pdf file of it. Please print the form, complete it and mail it to us at PO Box 5940, Lake Charles Louisiana 70606 or bring it by the church office at 617 West Claude Street. Our office hours are 8-12, 1-4 weekdays.
