Parent/Legal Guardian: I agree to the fullest exent possible, each participant and their family members shalll hold harmeless Our Lady Queen of Heaven, its affiliates, directors, and employees from and against all claims, damages, losses and expenses arising out of or resulting from the use of the center provided that such a claim, damage, loss or expense is attributable to bodily injury, sickness, disease or death.
